What is a Health Passport?

A health passport is a document that allows individuals to record details about their disability, health condition, or learning disability. The health passport helps individuals easily record information about their condition, any reasonable adjustments they may have in place, and any difficulties they face. Additionally, a health passport should be taken to every healthcare appointment and can help a person receive the best care by providing essential information such as medication, personal details, and communication preferences.
